A Defect Of Form For A Catholic Petitioner

This form should be completed if you were a Catholic at the time of the marriage, you failed to marry in the presence of a Catholic priest or deacon and did not receive permission from your bishop to be married by a minister or a Justice of the Peace, and you did not have the marriage validated or “blessed” at a later date.
